Covid-19: Daily Screening
Active screening is required for anyone entering Sarah's Family Daycare and it is the process of proactively checking for:
- symptoms (e.g., temperature checks and asking questions),
- travel history, and
- contact [without appropriate Personal Protective Equipment (PPE)] with a person(s) who may have COVID-19.
Parents should complete screening and temperature checks for their child, themselves and anyone in their household using the screening questions in the below form. After screening, complete the form and submit it.
When adults must enter the space (e.g., parents when necessary, people doing maintenance work, providing support, etc.,), they must also complete the screening before entering, however, they do not have to report their temperature.
Children and adults, when necessary, may enter Sarah's Family Daycare, when the results of the screening questions have been submitted and the answer to all screening questions are no.
